For AMOR Projects records, we need to keep in our files a copy of your immunizations for reference. If you have not given us that information, we will need to receive it before you serve with us. Usually, we recommend you wait for about 6 weeks before your trip to meet with your doctor and conduct a travel consultation. You can also check the CDC (Center for Disease Control) web site link for the latest recommendations on vaccines (you will be serving in the Ucayali region of Peru).
From the CDC Health Information for Travelers to Peru (latest update November 27, 2012):
Hepatitis A – 2 doses
Hepatitis B – 3 doses
Meningitis Booster at 16 years old
MMR – 2 doses
Polio – if OPV & IPV, total of 4 doses; if IPV, total of 3 doses
Tdap – after 10 years, Td booster, repeat booster every 10 yrs
Varicella (Chicken Pox) – 2 doses
Pneumovax – if Asthma, Diabetes – 1 dose
Typhoid – every 5 years booster after the Oral, Live, Attenuated Ty21a Vaccine
or Every 2 years after the Intramuscular injection of the Vi Capsular Polysaccharide Vaccine
Yellow Fever – the International Health Regulations (IHR) published by the World Health Organization (WHO) require revaccination at 10-year intervals.
Malaria prevention – Atovaquone-proguanil, doxycycline, or mefloquine
It is very important that you keep the Yellow Fever Immunization card with your passport.

The purpose of this form is to supplement, modify, update, or complete items previous submitted with the initial Student Missionary Application. You only need to fill out this form if you have been requested to by AMOR Projects administrators.
